Brief Paperwork

Our office intake form is now online and should be completed prior to your appointment. Upon entering the office, we have a few forms for you to read and sign. Filling out the intake form online prior to your appointment allows our staff to spend your entire appointment time with you and provides us with detailed information about yourself and your condition.

*All online intake forms must be competed on a tablet or computer and signed at the bottom with a stylus/mouse/finger in order to be complete.*

Consultation

Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent.


Diagnostic Testing

Our office uses the Myovision System to analyze and measure the stresses that your spine and nerves are currently exposed to. All new patients are scaned with the Myovision System on their first fisit and subsequently at various points of their follow up care to monitor the stresses in your spine and nervous systems. Additionally, your specific condition may require us to order x-rays or a MRI test to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. These tests are not performed at our office, but you will be provided with a prescription to obtain any testing that the doctor deems as necessary.

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include Chiropractic adjustments, active and passive therapies, and/or soft tissue therapies.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1 week and are provided with a complete review of findings if they have not already been provided. Treatment options for your condition will also be discussed during this time.
